Answer:

Q1Discuss the environmental and social factors linked to the increased incidence of diabetes in Australia.  Include in this response a statement about issues in the Australian health care system related to diabetes care delivery and diabetes related services e.g. Funding sources, increasing demand on services, access, and equity.
Environmental factors play a vital role in the etiopathogenesis of diabetes. This includes the unhealthy diet, air, soil, and water pollution, lack of improper physical activity. All these environmental factors trigger autoimmune responses among patients suffering from diabetes. Further, the physical and environmental factors also influence the behaviors and the choices of individuals suffering from diabetes. The proximity of the supermarkets encourages the individuals suffering from diabetes to consume fast foods and unhealthy food leading to a higher incidence of diabetes. The social factors linked with the higher incidence of diabetes in Australia include income, nutrition, education as well as access to housing. Poverty, as well as material deprivation, can be considered as the pre-requisites of increased incidence of diabetes in Australia. Moreover, chronic stress also leads to increased levels of depression, anxiety, lower energy and motivation as well as reduction of self-esteem.   With the increase in the demand for services, the Australian diabetes healthcare system has been witnessing a shortage of funding due to improper access and equity. Further, more than 1.5 million Australians affected by diabetes witnesses morbidity as well as mortality to a great extent. References Raman, P. G. Q2Discuss the social and environmental factors contributing to higher rates of diabetes in our (Aboriginal Torrens Strait Islander (ATSI) community?
The environmental factors that contribute to the higher rate of diabetes among the Aboriginal and the Torres Islanders include the unhealthy lifestyle, the lack of knowledge on the functioning of the heart and the blood vessel disease, to prevent obesity when they lead a traditional life. Moreover, due to genetics, the Aboriginals might have to face higher weight gain as well as other associated conditions such as heart disease and blood pressure. Water and soil pollution are also likely to affect the Aboriginals to a great extent. On the other hand, the social factors that contribute to the higher rate of diabetes among the Aboriginals include the consumption of unhealthy food that is low in fiber and high in fat, their sedentary lifestyle, smoking of a cigarette. With the advancement of technology, Indigenous Australians are subjected to poor physical activity leading to a higher rate of diabetes among them. Q6Briefly outline how the following challenges can impact the provision of primary health care services to Australian’s.
The ageing populationThe ageing population is likely to lead to increasing cost pressure with the rise in the healthcare and the social costs along with the expenditure on pension. The direct challenges of the ageing population in the Austarlain health system includes changing of the health profiles, the higher level of demand for health service usage as well as the rising health costs. There also has to be improved level of understanding as well as awareness on the health issues leading to higher level of expectations of the health services posing an additional challenge.
Cost of health technologyThe advanced health technology has raised the cost that is to be borne by the citizens in the primary health care centres. This has brought significant pressure in both the private and the public nursing homes thereby making it difficult for the citizens to avail the services. There has been unacceptable level of inequity in the health outcomes and the access to services and thus this has affected the primary health care services accordingly.
Health care fundingThe healthcare funding has posed a threat to the primary health care because of the growing burden of diseases, the workforce pressure and the ageing population. There has been unacceptable level of inequity in the health outcomes as well as access to the services in the primary health care. Moreover, as the funding is shared between the Government and the state territory, it becomes difficult to tackle the resources adequately.

Health inequality between indigenous and non-indigenous people

Health inequality has been known between the indigenous and non-indigenous population which is impacting the health care delivery system, most of the indigenous communities’ face lack of accessibility to the primary health care service. They is lack of culturally competent services, manpower and cultural liaison officer in many places which restrict the services delivery. Language, geographical and economical barriers also influence the health services further increasing the divide often reflected in the statistics indicating high rates of morbidity and mortality among indigenous people.

Q12Briefly outline the importance of critical thinking and problem-solving strategies are when determining appropriate health promotion and management plans are for patients when they discharge from hospitalSNYS
For an appropriate level of health promotion and management plans among the patients when they get discharged from the hospital, it is important to identify the key elements of critical thought i.e. problem-solving. This will make it easier to identify the problem, find out the probable cause of illness, and thus suggest the patient take the improved level of care.  Moreover, there will also be appropriate steps taken for the development of viable solutions to the problem thereby assisting the patients to adopt the correct strategy. Moreover, it can be said that critical thinking is essential for the management plans of the patients when they get discharged from the hospital so that they can be guided correctly and thus keep themselves away from such illness leading to hospitalization. It would also be easier for them to adopt preventive measures and thus begin to formulate independent judgments essential for critical thinking in the rapidly changing healthcare environment. Q13Briefly outline the importance of evidence-based practice and literature review in the development of primary health care-related policy and procedures by professional bodies associated with the management of primary health care.SNYS
The evidence-based practice in the development of primary healthcare policy and the procedure is important as it assists in the determination of the effective course of action that is essential to identify the problem. It also assists in the development of the improved level of evidence thereby analyzing the evidence and applying it to clinical practice. This is also essential to assess the results in the development of the primary healthcare-related policy thereby making it easier to share the values and the preferences accordingly. Moreover, the literature review can also be considered to be essential in the development of primary healthcare-related policies and procedures as it helps to summarise the previous knowledge, identifying the gaps of future research. Further, the literature review also acts as the base for educational research maximizing the relevance, impact, originality, and generalizability to a great extent. Q14As an enrolled nurse, you may find yourself working in primary health care. Briefly describe the ways you might contribute to health policy in that setting.SNYS
As an enrolled nurse, I will try to develop the health promotion policies that are associated with the primary healthcare setting. This promotion policy will cover the wider areas of illness prevention, public, and population health. It is also my responsibility as a primary healthcare nurse to provide treatment and care for sick individuals who are in the interdisciplinary teams. There should also be appropriate steps taken for chronic illness management thereby carrying out further research and advocacy in that particular area. It is also necessary to organize the clinical work environment including risk management as well as infection control. I must also try my best to solve the problems in the organizational and the clinical contexts associated with the primary healthcare setting. References IHNA (2018, May 22.)
